I hereby apply to become a lifestyle trainer of Join The Club Now & Real Wealth. As an lifestyle trainer, I understand and agree that: 1. I am of legal age (18 years or older). 2. I shall become an lifestyle trainer of Join The Club Now & Real Wealth upon acceptance of this application by the company in accordance with the companies membership Plan and Policies and Procedures. As an lifestyle trainer, I understand I will purchase products at regular customer prices. 3. The application becomes effective upon the acceptance of the signed application by Join The Club Now & Real Wealth. At that time Join The Club Now & Real Wealth will assign a unique lifestyle trainer Identification Number which must be used in all official business correspondence and contacts with the company. Join The Club Now & Real Wealth allows only one position per person. This may be one SSN or EIN, but not multiple entities or combinations of entities. 4. The company may amend the Policies, Procedures, and Compensation Program at any time without prior notice. 5. I MAY CANCEL PARTICIPATION IN THE Join The Club Now & Real Wealth LIFESTYLE TRAINER PROGRAM AT ANY TIME FOR ANY REASON UPON WRITTEN NOTICE TO Join The Club Now & Real Wealth. 6. Upon acceptance of the Application by the Company I will become an independent contractor responsible for my own business. Further, I will not be an employee of the company and will not be treated as such in regards to any laws including but not limited to the Federal Insurance Contribution Act, Federal Unemployment Tax Act, the Social Security Act, income tax withholding, or any federal tax laws. It is my responsibility to abide by all state, income, and tax laws. 7. I will not use the company's product, programs, and company names in any advertisement or promotional material not provided to me by Join The Club Now & Real Wealth or pre-approved by Join The Club Now & Real Wealth. 8. I must be a member in good standing with Join The Club Now & Real Wealth to retain an active lifestyle trainer status. 9. I understand that any and all commissions, bonuses, and/or overrides will come into my Paypal account only. 10. Join The Club Now & Real Wealth lifestyle trainers are independent contractors and are not purchasers of a franchise. This Application is not intended nor shall be construed to create a relationship of employer-employee, agency, partnership, or joint venture between the lifestyle trainer, sponsor, and/or Join The Club Now & Real Wealth. lifestyle trainers have no legal right or authority to bind Join The Club Now & Real Wealth to any obligation. 11. This Application will be processed when all information on this form is filled out and the appropriate fee is submitted to the company. 12. A Join The Club Now & Real Wealth membership (tuition) of $565.00 USD is required to participate in the Join The Club Now & Real Wealth lifestyle trainer program. 13. By virtue of this completed application, Join The Club Now & Real Wealth has permission to reuse, publish, reproduce, and distribute to the public any comments, testimonies, recordings, appearances, photographs, and any other medium in conjunction with your name if applicable, ("items") which have occurred as a result of your association with Join The Club Now & Real Wealth. Join The Club Now & Real Wealth is released and held harmless from any and all liability associated with or as a result of using these items. You may not be able to inspect or approve Join The Club Now & Real Wealth's finished use of these items. If you do not want these items used by Join The Club Now & Real Wealth, you must notify us in writing within 10 days of your application. 14. Upon registering you understand that if your Join The Club Now & Real Wealth membership lapses you will NOT be compensated for downline activity that particular month. It will restart on the next calendar month when you reinstate your membership. If your Join The Club Now & Real Wealth membership expires you will NOT be compensated for personal or downline activity. When you reactivate your Join The Club Now & Real Wealth.com Membership, you will also be required to pay a one time reinstate fee of $200.00 USD. If your membership is inactive for 10 days, you are required to pay a one time reinstate fee of $300.00 USD. After 30 days you will have to pay a reinstate fee of $500. After 60 days you lose your position but you may reregister again at the $565.00 membership fee. 15. I understand that I may sell memberships to anyone over age 18 and receive the stated residual bonuses for as long as they hold their membership. 16. I understand that Join The Club Now & Real Wealth members are NOT obligated to become lifestyle trainers. 17. Enroller signature or web application submission is required for any bonuses to be paid. 18. I acknowledge that I have read, understand, and agree to the terms set forth in this Application.
